You have been invited to a Covid vaccination clinic in Strand Medical Practice.

Please read before booking:

  1. Please click “submit” button only once.
  2. You will receive e-mail with confirmation of appointment after submitting the form below. You will be also redirected to a page with description of common side effects.
  3. Please send only one form and check your e-mail (including SPAM folder) before assuming the booking did not go through.
  4. Please note that below booking is based on HSE declaration to deliver sufficient amount of vaccines. Shall HSE fail to deliver, we might be forced to cancel appointments.
  5. If you can not fill in the form because some of required statements are not true in you case (for example if you had anaphylaxis in past), please call the surgery.
  6. If you changed your last name, please use the name you used to register with us, so we could identify you in our IT system.
  7. If there is no time option to be chosen and there is a note “no slots available” that means that no more appointments (or stanby appointments) are available on given day. New appointments will be made available as long as we have supply. Keep checking this website.
  8. Please choose only one option (so do not book appoitment and standby, pick just one). Otherwise it creates significant workload to delete double bookings.
  9. We will be using Pfizer vaccine.
  10. If you got your 1st and 2nd vaccination outside of this surgery you must present your vaccination certificate. Have it ready entering the building.
  11. You have to be at least 3 months after your second dose to receive a booster.
  12. If you had Covid after your second dose the soonest you can get a booster is 3 months after the diagnosis.
  13. Please try to book the first appointment available to help us avoid any vaccine going to waste.
  14. Patients who received Pfizer or Moderna for 1st and 2nd vaccination will not have to wait 15 minutes in the surgery. If this is your first time receiving mRNA vaccine, you will still have to stay in waiting room for 15 minutes.
